ROANOKE, Va. (WFXR) — A Roanoke nursing home has been cited by the Virginia Department of Health (VDH) for a deficiency relating to patient quality of care.
WFXR reported in January that VDH cited Raleigh Court Health and Rehabilitation Center for a deficiency related to resident records. We also reported at that time that a December onsite visit by VDH to the health and rehab center uncovered a deficiency related to the quality of care. However, at the time, the report was not publicly available.
VDH has now released the report from that December visit. The report states that staff failed to treat and assess a patient’s skin wound. The report concluded that “these failures constituted neglect of the resident that resulted in harm to the resident.”
WFXR also uncovered several search warrants related to alleged abuse and neglect at Raleigh Court. The search warrants refer to reports filed with the Roanoke Police Department (RPD) between Oct. 30, 2020, and Jan. 11, 2021, regarding the care of several residents.
A spokesperson with RPD states that the investigations are active, but no arrests have been made.
WFXR reached out to Raleigh Court Health and Rehabilitation Center. Chance Craft, administrator for Raleigh Court, provided the following statement:

Care Compare, a tool provided by the U.S. Centers for Medicare and Medicaid Services, lists Raleigh Court Health and Rehabilitation as a five-star care facility.
